Effective
1 January 2009 Atlanta Family Psychiatry's fees will
change to and be pro-rated at $120.00 for 30 minutes.(This
is the first rate change in approximately 10 years).
Our "late cancellation" and "no-show"
charge has also been changed to $120. AFPI regrets
the need to make this change.
Although
Dr. Ware does work with some insurance companies,
every effort is made to establish a financial plan
with any family that desires to use his services.
Those
interested in using insurance should contact their insurer
directly to determine if Dr. Ware is a listed provider for your
particular company. Dr Ware's billing and business office may be
able to assist.

Dr.
Ware’s rate is $120.00 per 30 min. Charges are prorated
for times over or under 30 minutes.
Clients
are encouraged to consider “out of pocket” payment which should
be discussed directly with Dr. Ware. See Section D of the New
Client and Payor Information Questionnaire (Welcome page, New
Clients section).
Payment is due
at the time of service.
Finance charges (1 ˝ percent of the unpaid balance per month/18%
APR) are applied to balances remaining over 60 days unless
payment arrangements are made with the billing office or Dr.
Ware prior to 60 days.
The client is responsible for all deductibles and charges not
covered by insurance.
When cost
is a significant issue, the client is encouraged to discuss this
with Dr. Ware.
